Esta visi\u00f3n sist\u00e9mica \u2014coherente con los modelos contempor\u00e1neos del movimiento y el dolor (Treede et al., 2019; Cohen et al., 2021)\u2014 nos recuerda que un s\u00edntoma no emerge de un solo tejido, sino del comportamiento global del sistema.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Sin embargo, dentro de esa complejidad existen situaciones donde el origen del s\u00edntoma <strong>s\u00ed es principalmente mec\u00e1nico y tisular<\/strong>. Es aqu\u00ed donde aparece el concepto de <strong>s\u00edntoma biomec\u00e1nico<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\" style=\"margin-top:var(--wp--preset--spacing--30);margin-right:var(--wp--preset--spacing--70);margin-bottom:var(--wp--preset--spacing--30);margin-left:var(--wp--preset--spacing--70)\"><strong>\u00bfQu\u00e9 es un s\u00edntoma biomec\u00e1nico?<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Un <strong>s\u00edntoma biomec\u00e1nico<\/strong> es una molestia o sensaci\u00f3n que se explica por <strong>c\u00f3mo los tejidos responden a la carga mec\u00e1nica<\/strong>. Esta es una idea ampliamente respaldada por la biomec\u00e1nica musculoesquel\u00e9tica (Nordin &amp; Frankel, 2012; McGill, 2016) y por la literatura cl\u00ednica tradicional.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">En t\u00e9rminos simples:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Es un s\u00edntoma que aparece cuando un tejido (m\u00fasculo, tend\u00f3n, c\u00e1psula, fascia o articulaci\u00f3n) es sometido a tensi\u00f3n, compresi\u00f3n o fricci\u00f3n, y responde con dolor o malestar <strong>local<\/strong>, <strong>predecible<\/strong> y <strong>directamente relacionado con el movimiento o la carga<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Este tipo de s\u00edntoma forma parte del dominio del <strong>dolor nociceptivo mecano-dependiente<\/strong>, descrito por la IASP como aquel con origen en la activaci\u00f3n de nociceptores perif\u00e9ricos frente a est\u00edmulos mec\u00e1nicos (Treede et al., 2019; Woolf, 2011).<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\" style=\"margin-top:var(--wp--preset--spacing--30);margin-right:var(--wp--preset--spacing--70);margin-bottom:var(--wp--preset--spacing--30);margin-left:var(--wp--preset--spacing--70)\"><strong><strong>\u00bfC\u00f3mo se reconoce un s\u00edntoma biomec\u00e1nico?<\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">La evidencia cl\u00ednica muestra patrones muy consistentes (Shacklock, 2005; Reiman &amp; Manske, 2018):<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Localizaci\u00f3n anat\u00f3mica clara<\/strong>: el malestar se siente en un sitio espec\u00edfico.<\/li>\n\n\n\n<li><strong>Reproducibilidad mec\u00e1nica<\/strong>: el mismo movimiento o postura siempre lo activa.<\/li>\n\n\n\n<li><strong>Relaci\u00f3n carga\u2013s\u00edntoma lineal<\/strong>: m\u00e1s carga \u2192 m\u00e1s s\u00edntoma; menos carga \u2192 menos s\u00edntoma (Barton &amp; Lack, 2017).<\/li>\n\n\n\n<li><strong>Mejora al cambiar la postura o reducir la tensi\u00f3n<\/strong>, lo que confirma su origen mec\u00e1nico.<\/li>\n\n\n\n<li><strong>Sin parestesias ni signos neurol\u00f3gicos<\/strong>, lo que lo diferencia del s\u00edntoma neurobiomec\u00e1nico o neurop\u00e1tico (Nee et al., 2012).<\/li>\n\n\n\n<li><strong>Patr\u00f3n estable en el tiempo<\/strong>, con baja variabilidad diaria.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Estos elementos indican que el s\u00edntoma <strong>tiene sentido mec\u00e1nico<\/strong> y depende principalmente del comportamiento tisular.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\" style=\"margin-top:var(--wp--preset--spacing--30);margin-right:var(--wp--preset--spacing--70);margin-bottom:var(--wp--preset--spacing--30);margin-left:var(--wp--preset--spacing--70)\"><strong><strong><strong>\u00bfPor qu\u00e9 ocurre?<\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Los tejidos tienen propiedades viscoel\u00e1sticas y responden a las cargas mec\u00e1nicas de manera predecible (Nordin &amp; Frankel, 2012):<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>si la tensi\u00f3n es excesiva \u2192 aparece tirantez o dolor localizado,<\/li>\n\n\n\n<li>si hay compresi\u00f3n prolongada \u2192 surge molestia puntual,<\/li>\n\n\n\n<li>si hay fricci\u00f3n repetitiva \u2192 puede generarse irritaci\u00f3n,<\/li>\n\n\n\n<li>si las cargas se sostienen demasiado tiempo \u2192 aparece rigidez o sensaci\u00f3n de \u201cfatiga tisular\u201d.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Muchos s\u00edntomas comunes -como la molestia cervical al mantener una postura prolongada, o el dolor localizado en un tend\u00f3n tras una tarea repetitiva- responden exactamente a este mecanismo (Punnett &amp; Wegman, 2004).<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\" style=\"margin-top:var(--wp--preset--spacing--30);margin-right:var(--wp--preset--spacing--70);margin-bottom:var(--wp--preset--spacing--30);margin-left:var(--wp--preset--spacing--70)\"><strong><strong><strong>\u00bfPara qu\u00e9 sirve distinguirlos?<\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Diferenciar un s\u00edntoma biomec\u00e1nico de uno neurobiomec\u00e1nico o neurop\u00e1tico <strong>cambia la intervenci\u00f3n<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Los s\u00edntomas biomec\u00e1nicos suelen mejorar con estrategias centradas en:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Modificar la carga<\/strong>,<\/li>\n\n\n\n<li><strong>Variar el movimiento<\/strong>,<\/li>\n\n\n\n<li><strong>Mejorar la tolerancia tisular<\/strong>,<\/li>\n\n\n\n<li><strong>Optimizar el control motor<\/strong>,<\/li>\n\n\n\n<li>y <strong>reorganizar la distribuci\u00f3n de esfuerzos<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">En cambio, los s\u00edntomas donde participa la sensibilidad neural requieren intervenciones m\u00e1s amplias, que incluyen tambi\u00e9n educaci\u00f3n, modulaci\u00f3n neurodin\u00e1mica y trabajo sobre la percepci\u00f3n corporal (Shacklock, 2005; Woolf, 2011).<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\" style=\"margin-top:var(--wp--preset--spacing--30);margin-right:var(--wp--preset--spacing--70);margin-bottom:var(--wp--preset--spacing--30);margin-left:var(--wp--preset--spacing--70)\"><strong><strong><strong>En resumen<\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-top:0;margin-right:var(--wp--preset--spacing--70);margin-bottom:0;margin-left:var(--wp--preset--spacing--70)\">Aunque el SMC funciona como un sistema complejo donde intervienen m\u00faltiples dimensiones, los <strong>s\u00edntomas biomec\u00e1nicos<\/strong> representan uno de los modos m\u00e1s directos en que los tejidos expresan su respuesta a la carga.
